Oak Creek Hospice is a family, owned Hospice in Folsom California, servicing all communities within 60 miles from our location. We provide skilled home care with a variety of services with compassion and knowledge to provide quality of life for patients and their families. Our dedicated team consist of Registered Nurses with over 30+ years of experience, Vocational Nurse with excellent wound care experience and Experienced Compassionate Home Health Aids. Our team delivers coordinated care with our nursing team and Social Workers that are knowledgeable of resources in our community to better assist patients with their psychosocial needs. Finally, our Spiritual and Bereavement counselors are uplifting and aware of all religions, practices, and preferences and are available to minister our patients and families throughout this difficult time. Oak Creek Hospice offers a unique one-of-a-kind volunteer program known as “HOSPITALITY” which provides activities such as tabletop games, arts and crafts, music, magazines, and inspirational tapes along with reminiscence material and scripture reading. This creates positive changes in each person’s mood and emotional state by decreasing isolation through the involvement of one-on-one activities.
