Who Provides Care?
Hospice care is delivered by our skilled team of professionals who work within the patient’s needs on medical, physical, emotional, and spiritual levels. Our team consists of the patient’s attending physician, hospice medical director, registered nurse, home health aide, social worker, chaplain, and trained volunteers who are ready to serve.
Our Caregivers Are Trained to Provide Specialized Care and Support
Skilled Nursing
Our Registered Nurses (RN’s) and Licensed Vocational Nurses (LVN’s) provide nursing care under the direction of the patient’s primary care physician or referring physician, providing excellent wound care and pain management.
Medical Social Services
Our Medical Social Workers assist patient’s and patient’s families to ensure that the psychosocial needs are met. We assist with any paperwork or outreach to community resources that our patients or families might need during this difficult time.
Volunteer Services
Our Volunteers provide emotional support, compassion, and positive energy to patients and family or caregivers. We have a company designed Hospitality program which includes activities such as conversing with patients, reading to, or playing games with patients, assisting with light housework or errands, as well as listening to music and going over reminiscence material.
Medical Director
Our Medical Director works with the patient’s attending physician and the hospice team, to provide guidance and leadership on the use of medicine in our healthcare setting, as well as devises the protocols and guidelines for our clinical staff.
Spiritual Services
Our Chaplains provide an ear to listen to patients and family members as well as give support with spiritual issues and provide emotional healing and spiritual growth.
Bereavement Services
Our Bereavement Coordinator provides counseling and bereavement support to family and caregivers. They continue to support the immediate family following death.
